    Left side is the 12 range candles and the right is the 4 range

    On the left he has drawn what looks like a pitchfork (also called andrews pitchfork) so from looking at the slope of that pointing upwards he has determined a bullish bias

    On the right he has the smaller time frame and has drawn a trendline (grey line) sloping downwards which when that breaks he will enter the trade and then look to scale out of his position once the previous high on that 4 range chart has been broken.

    This is after he has entered the trade you can see the entry was at the trendline break and the stop was near the previous swing low on that chart

    The target is set to scale out 9 contracts at the previous high on that chart leaving 1 to run increase it move 20+ points.

Yes it still works i know loads of people that trade purely from reading the tape, in answer to your first concern - it comes with experience and knowing which to focus on depending on the action. Yes the CME introduced the rule but if you look on the ladder you will still see fake offers and bids occurring because frankly how can anyone prove they were faking the order? Maybe they wanted to get filled but someone traded 1000 lots and they didnt like what they saw so they pulled their order, was the order legit? At that time yes it was they wanted to get filled. These rules can always be worked around. And i would say the main part of most of the setups are icebergs (for accumulation) followed by aggressive market orders to move the price in their favour.
